Most insurance policies require you to pay a deductible and/or co-insurance. In addition, you could be responsible for non-covered services. Please contact your insurance carrier for specific answers to your questions. You are responsible for any balance unpaid by your insurance.

Insurance plans do not always cover all services. You can find out what is covered and what is not by referring to your health benefits manual provided by your insurance carrier or employer. If your claim has been denied, please contact your insurance carrier for specific details.

What services must be pre-certified?

Since insurance carriers vary, please contact your insurance carrier for specific instructions concerning pre-certification. If an insurance carrier's pre-certification policy is not followed, many carriers impose a financial penalty to the patient or in some cases refuse to pay at all.
